Tennessee Department of Correction Commissioner Frank Strada is no stranger to botched executions such as the one in which defense attorneys say death row inmate Tony Carruthers was “tortured.”

Strada served as deputy director of the Arizona Department of Corrections in 2022 when the state ran into problems inserting IVs during three executions there.
